COBRA General Notice

On May 2, 2014, the Department of Labor (DOL) issued new proposed regulations to the COBRA General Notice requirements. Because the regulations are proposed, they are not final per se. Until the proposed regulations are finalized and effective, the DOL has stated that it will consider use of the newly revised model notices as good faith compliance with any COBRA notice content requirements. Rehn & Associates will provide an updated notice when the regulations are final.

COBRA Qualifying Event Form

This form is to be used by Employers to submit notification to Rehn & Associates of a COBRA qualifying Event. Employers are required to notify Rehn & Associates within 30 days of the loss of coverage.

COBRA Insurance Premium Information Form

Employers are required to keep our office notified of any and all carrier and benefit changes. Please use this form to notify our office of carrier and premium information.
