COBRA General Notice

The COBRA General Notice, also known as the Initial Notice, is a required communication that informs employees and their covered dependents of their rights to continue health coverage under COBRA. It must be provided within 90 days of the employee or dependent first becoming covered under the group health plan.

COBRA Qualifying Event Form

The COBRA Qualifying Event Form is used by Employers to notify Rehn & that a specific qualifying life event has occurred—one that may trigger eligibility for COBRA continuation coverage. Employers are required to notify Rehn & Associates within 30 days of the qualifying event.

COBRA Premium Rates & Information Form

The COBRA Premium Rate & Information Form is an essential document, required annually from Employers. This form should reflect updated premium costs, coverage tiers, plan changes, etc. for individuals continuing health coverage after leaving a job. Timely and accurate data supports compliance with federal COBRA laws, reduces administrative errors, and ensures clear, consistent communication with qualified beneficiaries.
